Requirements & Prerequisites
Proof of Immunity for Measles, Mumps, Rubella (MMR) AND Varicella. This can be completed via blood titer (depending on immunity, a booster shot may be required)
Proof of Childhood Immunizations/Vaccinations records
Covid19 vaccination card OR Declination along with Exemption letter from your local pastor/Primary Care Physician.
Social Security Card
Valid Driver’s License OR Government issued photo ID
American Heart Association CPR Card (this will be completed during the program)
Mask fit test (offered monthly at OMI, or can be completed at Advent Health)
High School Diploma OR G.E.D (letter of attestation from High School can be accepted as well )
Flu Shot (Seasonal – during Oct. 1st — April 30th )
Current School Physical (dated no later than 6 month from the start date of class)
10 Panel Drug Screening
Background Check completed Via PreCheck https://www.precheck.com/students
TB/PPD Test with results (dated no later than 6 month from the start date of class)
